The Department of Housing is committed to using technological resources to make our processes easier for resident use and more efficient for all. This page is where all forms and applications are housed. Most of these are time-sensitive and will be placed here as procedures and projects occur throughout the year. The Maintenance Request/Status allows residents to inform the Facilities unit of a room or building-based concern or outage. This request is routed to the appropriate trade or shop, so please complete all sections of the form. The ResNet Help Request is for reporting problems with your room network, telephone or television service. The Have a Question form is used to request additional information from any component of the Department. An email response will be sent to you from the appropriate office or unit.
